Position Summary:
As part of the Recuperative Care Program (RCP) Team at the 40 Prado Homeless Services Center this position will work with up to 6 homeless individuals needing 24-hour supervision due to recent discharge from a participating hospital or skilled nursing facility.
The RCP Nurse will monitor safety and stability of participants, coordinate services with RCP Case Manager, staff, and other appropriate professionals, including home health and on-site CHC medical staff to ensure continuity of care, safety, and gradual transition back into the community.
Together with RCP Case Manager, will facilitate/provide transfer of appropriate individuals to 40 Prado from hospitals and work with staff to provide participants with needed services aimed at stabilizing their physical health needs.
Supports CAPSLO’s mission by providing medical assistance to individuals seeking to achieve self-sufficiency.
Essential Functions: 1.
In coordination with CAPSLO RCP Case Manager and Deputy Director, determine appropriateness of referrals from hospitals (must be independent in all ADL’s).
When appropriate, interviews patients at hospital and coordinates with nursing staff/hospital discharge workers to ensure smooth discharge as schedule allows.
2.
Oversees RCP staff charting to ensure accurate and timely documentation of vitals and medication storage.
3.
Disseminates information on nutrition, physical activity, tobacco/alcohol/drug use, functional status at home, and all other health and lifestyle issues.
4.
Reviews current medications of participants and advocates for participants to ensure continuity of treatment.
5.
Addresses weight, reviews eating habits and exercise levels.
6.
Educates and counsels on physician visits, assisting the client to understand and advocate for their health.
7.
Works with RCP Case Manager to refer participants to physician or appropriate provider such as health clinics and community health services when screening reveals a potential medical condition.
8.
Gives special attention to detection of depression symptoms, suicide risk factors, signs of physical, mental and financial abuse or neglect or change in cognitive functions.
Reports as required by law.
9.
Provides training to RCP staff on applicable medical/medication issues as they relate to clients in the program.

Knowledge of general nursing and charting principles.

Knowledge of local human services delivery system.

Must be computer literate with basic knowledge of Microsoft Word, Excel and data tracking programs.
Will be expected to learn Homeless Management Information System (HMIS) within 30 days of hire.
Skills 1.
Must be proficient in charting and other documentation required.

Ability to meet and deal pleasantly and effectively with homeless individuals.

Supervisory Responsibility: This position has no direct supervisory responsibilities.
Work Environment: Requires working indoors in a temperature-controlled environment with some exposure to copier and printer chemicals/fumes.
The work environment may include some exposure to aerosol transmissible diseases (ATD).
As part of an employee health and safety program, personal protective equipment is provided as needed.

Travel: Some travel may be required to attend meetings, conferences and trainings.
Required Education and Experience: Must be a California licensed Registered Nurse or Licensed Vocational Nurse, CPR certified, and two years of experience in a hospital environment, counseling, disseminating information on availability of preventative measures and tests in the community.
Preferred Education and Experience: Experience working with the homeless is preferred.
License & Certifications: 1.
Current California licensed Registered Nurse or Licensed Vocational Nurse.
2.
Current CPR certification.
3.
Must have dependable insured transportation, and a valid California Driver License (mileage to be reimbursed) and acceptable driving record.
A DMV printout and proof of insurance will be required.
Additional Eligibility Qualifications: 1.
Must be at least 18 years of age by date of hire.
2.
Must submit proof of current vaccination status for measles, pertussis, and influenza (or a current, signed influenza declination form) in compliance with SB 792.
3.
Must complete a health screening physical completed within 12 months prior to employment or 7 days following the date of hire, and a current TB skin test/Chest X-Ray.
4.
Employment is contingent upon meeting all job requirements and background requirements: criminal history check (Inclusive of DOJ, FBI and Child Abuse), acknowledgement of child abuse reporting responsibility, criminal record statement (Form LIC508), etc.
A criminal history clearance is required within seven days of completing the Live Scan.
5.
Upon hire agency staff will check the Megan’s Law (or the current sexual predator website) to ensure each staff being hired is clear for employment.
Program administrative staff will check the website annually to ensure all program staff are cleared to work.
