Position Summary: As a member of the Services Affirming Family Empowerment (SAFE), the Family Advocate will coordinate referrals, participate in School Student Study Team and weekly case management meetings.
The Family Advocate will conduct needs assessment, assist in development and follow-up of case plans and provide additional early intervention through in-home support.
The Family Advocate will also provide a realistic framework for families that participate in the program, which focuses on prevention and early intervention.
Minimum Educational and Experience Requirements: • Family Advocate I: • Bachelor’s Degree in Child Development, Family Studies, Psychology, Social Work, or Health Education or equivalent experience.
• Family Advocate II: • Bachelor’s Degree in Child Development, Family Studies, Psychology, Social Worker, or Health Education and two years related experience, or an equivalent combination of education and experience.
Responsibilities and Duties: • Facilitate entry of eligible children into the SAFE programs, and orient parents to the goals and objectives of SAFE.
• Support SAFE families by helping to identify problems and concerns, and by linking families with appropriate referral systems and services.
Follow-up with families to ensure that services are received and encourage families to obtain self-sufficiency skills.
• Keep informed of community agencies, resources, workshops and classes; encourage parent participation in program and community events.
• Obtain pertinent medical and education information, including attendance and achievement data, needed for documentation of special needs children for the multidisciplinary team’s monthly review, while maintaining confidentiality at all times.
• Conduct regular home visits, as needed, to help open communication and develop a trusting relationship with the family.
• Participate in a comprehensive and extensive family needs assessment in order to develop family case planning to ensure that the multidisciplinary team obtains information necessary to develop case plans that will focus on the whole child and his/her family.
• Ensure that the Case Plan is initiated, modified as needed, and the family understands the purpose and intent of the Plan.
• Participate in the Student Study Team to identify students at risk for school or community problems.
• Under the supervision of the Site Coordinator and in collaboration with SAFE partners, provide case management services such as, maintaining daily living skills or developing social skills.
• Encourage parent participation at all school conferences, parent education nights, and at the Individualized Educational Plan meetings.

• Ensure that SAFE families feel their ideas are validated and implemented throughout the problem-solving and decision-making process.
• Provide one-on-one case management on a continuous basis to implement health, nutrition, school completion/equivalency, child development and parenting goals.
• Facilitate support groups and parenting classes.
• Communicate with and refer to Mental Health staff about psycho social, medical and case management issues.
• Network with related agencies in linking clients to community resources.
• Perform other duties as assigned.
Other Requirements: • Must have knowledge of general office skills • Must be able to read and write with good communication and comprehension skills.
• Must have the ability to coordinate and communicate with parents.
• Must have the ability to relate well and establish rapport with students and parents.
• Must be able to work cooperatively with staff, community agencies and persons in the childcare field.
• Must be comfortable in making home visits.
• Computer knowledge and skills desirable.
• Must complete a TB skin test or chest x-ray prior to beginning employment.
• Must have a valid California Driver’s License, and a reliable vehicle available for business use (mileage reimbursed) and acceptable driving record.
• Will require frequent travel within the South San Luis Obispo County area with occasional travel to Central Headquarters in San Luis Obispo.
• Must be willing to work evenings and/or weekends, as needed.
• Must follow all required child abuse reporting requirements.
• Must be able to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, veteran status, sexual orientation, disability or socio-economic level.
• Must use reasonable precautions in the performance of one’s duties and adhere to all applicable safety rules and practices; and act in such a manner as to assure at all times maximum safety to one’s self, fellow employees, clients and children.
• Employment is contingent upon meeting all job requirements and background requirements: criminal history check, education verification, acknowledgement of child abuse reporting responsibility, criminal record statement, etc.
Employment is contingent upon receiving a clearance from appropriate authorities.
• Must provide a certificate of completion of an approved first aid class to be evidences within 90 days of hire.
• Must understand, uphold, and continue working toward accomplishing the mission, strategic goals and performance measures of the agency.
Physical and Environmental Requirements: • Position requires intermittent sitting, standing, walking, twisting and bending.
Must be able to lift up to 30 pounds.

• Simple grasping and hand manipulation required, as well as reaching above and below shoulder level.
• Must be able to withstand exposure to noise in a day care environment.
• Must be able to drive to meet families, providers and children.
Additional Eligibility Qualifications: • Must be at least 18 years of age by date of hire.
• Must submit proof of current vaccination status for measles, pertussis, and influenza (or a current, signed influenza declination form) in compliance with SB 792.
• Must comply with the CAPSLO COVID-19 Vaccination Policy, either being fully vaccinated or sign a COVID-19 vaccination declination form.
• Must complete a health screening physical completed within 30 days prior to employment and a current TB skin test/Chest X-Ray.
• Employment is contingent upon meeting all job requirements and background requirements: criminal history check (Inclusive of DOJ, FBI and Child Abuse), acknowledgement of child abuse reporting responsibility, criminal record statement (Form LIC508), etc.

Program administrative staff will check the website annually to ensure all program staff are cleared to work.

CAPSLO pays the full amount (100%) of the premiums to cover full-time employees, and half (50%) of the premiums to cover eligible part-time employees.
