Description:
The Health Coach (HC) collaborates with our clinical team on developing, facilitating, training, and educating patients with chronic respiratory disease using virtual/remote technology.
This position plays a key role in our Virtual Pulmonary Rehabilitation program.
The Health Coach will focus on motivating patients towards self-improvement and goal setting all through education within our program.
This position will also help with additional patient needs such as answering inbound calls, medication refills, health risk assessments, and care calls to help provide an excellent patient experience.
This is a full-time, onsite position at our San Luis Obispo, California office that reports to the Chief Operating Officer.
The pay scale for this position depends on the candidate’s level of certification.
The pay scale for Health Coaches is $25-$35/hour.
Responsibilities:
Connect, support, and partner with patients to ensure their success in our Virtual Pulmonary Rehabilitation program
Provide patients with education and exercise programs
Collaborate with the clinical team to improve patient outcomes
Telephonic outreach to patients regarding programs or services for which they are eligible to receive
Documents clear and concise notes in the medical record
Complies with all company policies, federal, state, and local legal and certification requirements, as applicable
Requirements:
Education required:
Health and Wellness Coach Certificate, required
National Board Certification, preferred
Bachelor’s Degree in any health related field, preferred
Strong knowledge of geriatrics is a plus
Experience required: 1 year working as a certified health and wellness coach
Skills/Knowledge/Abilities:
Knowledge of chronic respiratory conditions (i.
e.
, COPD, Asthma)
Experience with respiratory and exercise
Strong communication and facilitation skills
Ability to use video conferencing platforms (e.
g.
, Microsoft Teams etc.
)
Strong verbal and written communication, and presentation skills
Detail-oriented, organized, and self-motivated, with the ability to learn quickly
Must be passionate about contributing to an organization focused on continuously improving patient and client experience & outcomes.
Experience with Pulmonary Rehabilitation in the outpatient and/or virtual setting is a plus
Bilingual in English and Spanish preferred
